When you acces your personalized link 72 hours before you travel, you will need to provide the following information to complete the additional information for your e-visa:
- Full name
- Date of birth
- Gender
- Country of birth
- Passport issuing country
- Passport number
- Country of permanent residence
- E-mail (optional)
- Phone (optional)
- Arrival date in Cuba
- Airline name and flight number
- Point of entry to Cuba
- Country of origin of the trip
- Reason for the trip → Always Select “Tourism”
- The address of the place of accommodation in Cuba, including the municipality and the province.
- Countries visited in the last 15 days (optional)
- Disease symptoms (optional)
- COVID Questionnaire (optional)
- Immunization status (optional)
- PCR test data (optional)
- Unaccompanied baggage → Does not apply to roreigners; select “NO”
- Number of accompanying children
- Currency declaration → Only if you have more than $5000 on you.
- Importing Non-Personal Items (optional)
- MINCOM Technical Authorization Number for Import → Leave blank if not available.
- Customs declaration items → Only if you have something to declare.
Please note that:
- When the process is complete, a unique code will be generated. You can save this code. This code is required if you want to modify your form in the future before you travel. Save it to your phone and you will be presented with a screen similar to the one below.
- When the form is finished, you will receive a PDF similar to the one shown below. This document is your tourist card and you must keep it in your phone to present it to the Cuban authorities when entering the country.
